xmlroff was doing the right thing, but since the convention for
processing of the enumeration tokens had since changed (#58), I had
thought it was wrong.
The enumeration token handling is now that little bit more modern, but
it still doesn't produce small caps for me with either backend, though
AFAICT, I also don't have any fonts with small caps on this computer.
