I have no expectation that they would be the same.

Since they both use Pango, your idea that they should be the same would
be more reasonable.  Pango doesn't have a device-independent backend, so
xmlroff has to use the particular backend to get a PangoLayout, so I'm
not too surprised that they are different.

Feel free to create a new ticket for this.

There is #112, which is about Cairo changing how many lines it needs.
